If more than one state may be involved—for example, you moved after purchase—describe that in your submission. The department can tell you whether it has authority or whether another regulator should receive the matter.

Try the insurer, then preserve the trail

The NAIC advises consumers to try resolving the issue with the insurer first. Ask for a written response and record the date, channel, representative, and reference number for each contact.

Escalate when the issue remains unresolved or involves matters such as an unfair delay or denial, failure to honor the policy, a possible violation of state insurance law, poor communication, or disputed cancellation or nonrenewal.

Build a complaint file

  • The full legal name of the insurer or agent and your contract or policy number.
  • A dated timeline written in neutral, specific language.
  • The relevant contract pages, statements, illustrations, letters, emails, and call notes.
  • The insurer's response and the concrete outcome you requested.
  • Copies rather than irreplaceable originals, unless the department instructs otherwise.

Do not include unrelated sensitive records. Follow the department's secure submission instructions for personal or financial information.
